Licensing Program Analyst (LPAs), Odom and Nguyen conducted an onsite inspection for the purpose of an Annual Random. LPAs and Lead Teacher toured the facility inside and outside the floor and yard plan. Census was taken in classroom 57. At 12:09pm the overall census observed was 1 preschool staff and 18 preschool age children with 2 parent volunteers. LPAs observed 1 preschool age child walked into the classroom alone and Lead teacher stated child was in the bathroom located outside the classroom. At 12:20pm Staff #2 walked into the classroom and stated they took a bathroom break. During the inspection it was determined the facility is operating within its licensed capacity. Classroom 57 has an AM and PM half day program from 8:00a.m.- 11:00a.m. and 12:00pm to 3:00pm, Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; breakfast, and lunch are provided. Food prep areas were clean and sanitary. Food is properly stored. Menus were posted where they could be reviewed by parents. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by sport water bottles with each child’s name on it. The children's bathrooms are clean and sanitary. The facility has conducted an emergency drill within the past six months. At 12:25pm Lead teacher tested the carbon monoxide detector, and it was not in service. The fire extinguisher was charged and in compliance. Facility met all posting requirement.

The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material rubber mats commercially produced for the purpose around the climbing equipment, slides and other similar equipment appeared to be enough to absorbs falls. Drinking water in the outdoor activity space is provided by sports water bottles with each child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair.

From 12:00pm to 1:00pm childcare staff and LPAs attempted to reach district personnel to obtain personnel files. Staff files were not available for review during require visit however, immunizations as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a childcare center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for staff were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 03/2023.

From 12:00pm to 1:00pm childcare staff and LPAs attempted to reach district personnel to obtain children’s files. Children's records were not available for review during required visit; however, children’s emergency contact information was available in the classrooms. Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day.

The licensee was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Based on LPAs observations, record reviews and interviews the following violations were observed are being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 3, Section 101229(a)(1) Responsibility for Providing Care and Supervision, 101216.3(a) Teacher-Child Ratio, Health and Safety Code 1596.954, 101217(d) Personnel Records, 101221(a) Child's Records are being cited on the attached LIC 809D.

LPAs Odom and Nguyen informed facility representative Laura Cux that this report dated 1/12/2023 documents 2 Type A citations which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPAs Odom and Nguyen informed the facility representative to provide a copy of this licensing report dated 1/12/2023 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
