32 | During the staff interview, Staff #2 (S2) and Staff #3 (S3) disclosed when C1’s mother left the diapers at the facility for the teachers to change C1 throughout the day, C1’s mother labeled C1’s diapers with small number from 1 to 5 on the back of the diapers. The mother labeled the diapers to track if the teachers actually changed C1’s diapers. Staff 2 and 3 also stated when the teachers changed C1’s diapers, the teachers just randomly took any diaper and did not follow the diaper ordering number, such as 1,2,3,4,5. There was a day C1 wore diaper #1 and the teacher happened to pulled the diaper labeled #1 and changed C1. The mother complained to the director that the staff did not change C1’s diaper since C1 came home with the same diaper #1.
All interviewed staff denied of witnessing any staff members not changing the children’s diapers for the whole day. All interviewed staff confirmed that when the children wet or soiled their diapers, the staff members always wipe the children before putting on the cleaned diaper.
LPA Nguyen reviewed the diaper changing log and it indicated the facility staff changes the children every 3 hours or before nap time.
LPA Nguyen attempted to interview four children and all four children did not qualify for an interview due to being too young and non-verbal. Subject child is also too young to provide full statement.
LPA Nguyen contacted six parents by phone and was able to interview two parents. All two interviewed parents stated they like the facility and all staff members are very nice. Interviewed parents also stated they did not have any concern or issue with the facility.
Based on the information gathered from LPAs' interviews, reviewing records, there is insufficient evidence to corroborate the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the Staff did not meet day care child's diapering needs did or did not occur, therefore the allegation is UNSUBSTANTIATED.
Exit interview was conducted. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. “The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.” This report LIC 9099 was provided to the director. First level appeal is to Regional Manager, address is above on the report. |
