Complainant alleged that a child got hit, pushed, body slammed or even slapped in the face and none of these incidents were documented.
During the staff interviews, all 5 interviewed staff gave consistent disclosures regarding the facility's discipline policy, including talking to the child, notifying the director, and informing parents as needed. All five out of five interviewed staff stated they do their best to always keep the children safe. We were unable to determine who C1 is.
LPA Nguyen attempted to interview the children in care but only 5 were qualify for the interview. The five children interviewed stated teachers are very nice, and they like coming to school. No disclosures were made by children.
LPA Nguyen called 10 parents, LPA was able to interview 5 parents on 06/19/23. All parents interviewed made no disclosures, parents stated that the facility communicates with parents on what is going on with their child(ren) when needed.
Complainant also alleged that the facility does not report incidents to parents.
All 5 staff disclosed facility procedure to address an injury is to let the director know, then the director notifies parents via phone or at pick up time, depending on seriousness of injury.

During record review, the Parent Handbook, stated the injuries policy, which include parent will be called immediately if facility observed a head injury or in the event of any serious or questionable injury.
