An unannounced case management inspection was conducted on this date by Licensing Program Analyst (LPA) Stacy Torrence for the purpose of capacity increase. The facility has requested to add classroom #7(Kindergarten) to the school-age program license. The facility is currently licensed for 25 children and is requesting to increase to 38 children. LPA Torrence met with Director Rebecca Nehez, who guided analyst on a tour of the facility. Census was taken. There was one child present and one staff supervising. Operations hours are Monday-Friday; 6:30 a.m.-6:30 p.m. and 1 Friday per month; 6:30 a.m.-10:00 a.m.; serving children ages 4.9-12 years old. Waiver for shared playground on file.

A review of staff records on this date indicates that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.

All areas identified on the Facility Sketch were inspected. If children become ill during the day, they will rest in the isolation area located, in the director’s office. Medication is stored in the kitchen, in a lock box and in the Assistant Director’s office in a locked cabinet. Furniture and equipment were inspected for age appropriateness and good repair. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, paper towels, area safety and sanitation. Children bring their own water bottles from home, and there is a water jug available for the children to re-fill their bottles. The facility also disposal cups for the children. First Aid supplies were inventoried. Sign in / sign out record meets regulation requirements.

The facility provides breakfast, lunch, and AM and PM snack. Food prep areas appear clean and sanitary. Food is properly stored.

Indoor Activity Space Measurements:
Kindergarten (Room #7): 46.17 x 37.05 = 1,706.89 sqft. divided by 35= 48 children
School-Aged Classroom (Room #9): 45.03 x 23.08 = 1,039.29 sqft. divided by 35= 30 children

Total= 2,746 sqft. divided by 35 = 78 children

Total children toilets and sinks:
toilets 4 x 15 = 60 children

Outdoor Activity Space Measurements:
Total= 20,448 sqft. divided by 75= 272 children

Playground is completely enclosed. Outdoor activity area is supplied with age and size appropriate equipment. An adequate amount of cushioning material is in place under the equipment. Shade is provided by a canopy. Water is provided by water jug and disposal cups.

Fire clearance approval letter on file and dated 03/22/2021. Based on today's inspection, facility has enough space for the requested increase capacity of 38 school-aged children.

Center Director Rebecca Nehez required CPR/1st Aid training expires on 01/22.

Facility provides Incidental Medical services.

In the areas evaluated, no deficiency was observed and cited today per CA Code of Regulations, Title 22, and Division 12.

Exit interview was conducted. A copy of this report and appeal rights were discussed and provided to the director. The Notice of Site Visit was posted. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
