Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of the facility moving to a different building on this large campus by decreasing their capacity from 39 infants to 26 infants. The facility has been licensed for 39 children in rooms 110, 111, 112, and 114. LPA met with director, Jennifer Eggers who gave LPA a tour of the facility. The facility is located on the ground of Mariner's Church. There were 22 infants with 10 staff present today.
The facility has requested to provide care and supervision to 26 infants ages 4 to 18 months old, Monday to Friday from 7:00 am to 6:00 pm by moving to a new building on this existing campus (Building 7 in upper campus).
Parents sign in and out their children electronically by entering the lobby of this building and a staff person will escort the child to the classrooms behind a double door entrance.
A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The parents will provide morning, afternoon snack and lunch for their children. Food are broght from home must be labled with the child's name and current date on each item. The children are dropped off and are singed in and out at the entrance electronically.
All areas identified on the Facility Sketch were inspected. If children become ill during the day, they will rest in the isolation area located in a small separate room inside room 159 away from the other children, and use the adult restroom until sick children are picked up.
The new rooms in the new building was measured. There are two rooms designated for infants rooms with two designated nap rooms. The director was advised staff person must be present to supervise children while napping in this room. Children must be supervised at all times.

Following was observed in all the rooms:
Age appropriate furniture and activities including educational, arts & crafts items.
Rooms are well ventilated with proper lighting and shade.
According to director the smoke detector and Carbon Monoxide are combined. LPA observed the device installed in the ceiling of each room.

The measurement of the rooms according to updated numbers are as below:
Room 150 is: 513 square feet which is a separate nap room attached to room 151 and is not calculated in the activity space.
Room 151 is: 1080 square feet
Room 155 is: 465 square feet which is a separate nap room attached to room 159 and is not calculated in the activity space.
Room 159 is: 1086.05 square feet
Total indoor measurement for the rooms of 151, and 159 are 2165 square feet divided by 35 square feet =61.86 (62 children).
Toilets: 2 X 15= 30 children
Sinks: 4 X 15 = 60 children.
There in one sink located in room 150, and 151, and 2 sinks in room 159.
Outdoor Activity Space:
Total measurement of the outdoor space is: 1928.50 square feet divided by 75 square feet= 25.71
(26 children).
Playground is completely enclosed. Outdoor activity area is supplied with age and size appropriate equipment with adequate of cushioning materials such as turf. There are no climbing structure on this playground. The facility will bring out infants with outdoor toys to the outdoor space. Shade. Shade is provided by trees, and building structure.
LPA reviewed with director Infant safe sleep from (LIC 9227) and log for every 15 minutes check on infants during nap time, and the LIC 311A, Records to Be Maintained At The Facility, for child’s records,

personnel records, administrative records, and documents to be posted. The director's Immunization, Mandated Reporter Training, Health and Safety Training are current. Her CPR and 1st aid is EMSA approved and expires 7/29/2025.

The director, Jennifer Eggers was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA also informed the director, Jennifer Eggers of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA discussed the positioning of the changing table for visual observation of children while using it and being within the arm's reach of a sink. The diaper changing table has padded surface no less than one inch tick with washable vinyl. It has raised sides on four sides at least 3 inches high.


Storage cubbies are arranged to store personal belongings
Exit interview conducted and report was reviewed with the director, Jennifer Eggers.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Based on today’s measurements, the sinks and toilets availability, the request of the facility for moving to building 7 and decreasing their capacity to 26 infants in rooms 150, 151, 155, and 159 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required. Please send the following corrections within 30 days of today's date.


Fire clearance approval has been received in our office to approve the capacity of 26 infants. .
1- Fencing off the infant playground to measure 1928.50 square feet divided by 75 square feet= 25.71
(26 children). Please send a final picture after installing the fence to separate the playground from the preschool playground.
2- Exit gate on the playground opens to the parking lot by a push bar. A second burrier will be installed for not easy accessibility to outside on the parking lot. (Picture will be sent).
3- Removing low level mirror on the wall in room 150. The edges of the mirrors are sharp.
4- Final photo of the rooms after being set up since the facility has not moved completely.
5- Diaper changing tables shall be within the arm reach of a sink (Please send photo for each room).
