On 09/28/2022 at Licensing Program Analyst, Stella Gutierrez made an unannounced visit to the facility for the purpose of a complaint that was received for the above alleged allegation. LPA, Gutierrez met with Director, HeeJoon (Carolyn) Youn who was explained the purpose of today’s visit and provided a tour inside and out of the facility. LPA observed 53 preschool children napping on cots and 18 preschool staff providing care. A review of the Facility Personnel Report Summary conducted on 09/28/2022 indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.

A complaint was received regarding the allegation of physical plant- Staff did not ensure that the facility is free from pest. It was reported by the complaintant that a child in care had several spider bites over a course of a 1 week period. LPA, Gutierrez toured the facility grounds duirng visit today. Page 1 of 2

During today's inspection LPA interviewed director and obtained the following documents; LIC 9040 Children's Roster and the director's certificate of completion for Pest Management Practices that was completed on 10/15/2021.

During tour of the indoor accessible areas LPA did not observed any insects or hazards.

During tour of the outside play area LPA observed several living spiders, webs and spider eggs under 03 playground structures and under toddler bike. This area is an on limits area where the children play daily while in care. This poses a risk to the children in care. Director stated that she thoroughly checked the classrooms for any pest due to a verbal complaint from a parent of a child at the facility. Director also stated that she never thought to look outside on playground structures/equipment. Director did have maintenance come and trim the trees (in AM hours of today, 09/28/2022) in the outside area with the thought of insects coming from the tree branches.

LPA will provide additional resources to director via email regarding Healthy School Act Pesticides and requirements and hand out " Protecting your Child from Pesticides".

Based on LPAs observation and interviews the allegation of physical plant - Staff did not ensure that the facility is free from pest is deemed substantiated. A substantiated finding means that the complaint is substantiated, and the allegation is valid because the preponderance of the evidence standard has been met. The following violations were revealed and are being cited in accordance with Title 22, Division 12, Chapter 1, Article 07, Physical Environment 101238(a)(1). Deficiencies will be cited today Please refer to attached 9099 (D).

Exit interview conducted and report was reviewed with the director, Hee Joon (Carolyn) Youn . Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met

Director posted a sign on door of the outside off limits until the pest are removed. Director stated that she will provide pictures of the equipment once cleaned and free of insects to LPA via email @ stella,gutierrez@dss.ca.gov Director will also develop a plan on how regular cleaning maintenance of the outside area
