25 | On 8/6/24 at 11:00am Licensing Program Analyst (LPA) Sarah Garcia conducted a case management -licensee initiated visit to inspect the toddler 1 room and mutli-age room. LPA met with director, Jennifer Patterson. Director guided LPA on a walk through of the facility. LPA observed 22 infants with 7 staff. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 6/18/24, Director informed LPA that the facility will be initiating a change to the preschool and toddler program. The facility will be switching the toddler 1 room and mutil-age room. The infant capacity will remain 40 and the preschool capacity will remain 96. LPA received the facility sketch (LIC 999A) and STD 850 was approved by the City of Placentia Fire and Safety Department on 4/29/2024.
LPA inspected the updated toddler 1 room and observed the space to be clean and tidy. LPA observed a changing table and age appropriate toys and materials. LPA inspected the multi-age room and observed the space to be clean and orderly. LPA inspected the children's bathroom and observed it to be safe and operable. The room sizes are measured as follows: The toddler 1 room is 420 square ft and the multi age room is 560 square ft. LPA took photographs. LPA discussed regulations on indoor activity space with director.
File will be submitted to management for final approval.
Exit interview conducted and report was reviewed with the director, Jennifer Patterson. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Failure to post will result in Civil Penalties of $100.00.
