25 | On 10/16/23 Licensing Program Analyst (LPA) Aiddee Nunez conducted an unannounced Case Management due to deficiencies that were stated during an interview with Staff#1 (S1) on 9/5/23. Census was taken and there are 20 infant children, and 4 infant staff present during inspection.
A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
During an interview on 9/5/23, S1 stated that during the month of August 2023 there were 2 confirmed cases of hand, foot, and mouth at the same time. However, S1 did not reported it to Licensing and to the health department because S1 thought S1 had to report an outbreak when there are 3 or more cases at the same time. There is also no record of an outbreak being reported to licensing during the month of August 2023 from the facility.
Based on the information gathered from LPA's interviews and record reviews, the following violation was observed in accordance with California Code of Regulations, Title 22 Division 12 Chapter 1, Section 101212(d)(1)(E) is being cited on the attached LIC809D.
Appeal Rights and deficiency were explained. The Licensee was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the director Taryn Stephens . |
