32 | Regarding allegation 4: Licensee did not ensure proper ratios were maintained.:
During the staff interviews, 3 out of 3 staff interviewed stated the most children they will have in one day are 13 or less. S2 stated It’s between 5-9 children and usually the facility would have 7-8 older children and 3 small babies (infants). Licensee stated licensee takes care of older children and S2 takes care of younger children. LPA has conducted the inspections at the facility on 4/24/25, 8/6/25, 10/10/25, and 11/17/25, and licensee has been in compliance with the ratio and capacity requirement.
On 11/17/25 LPA reviewed files for the 13 enrolled children in facility and facility was in compliance with the regulation requirement.
Regarding allegation 5: Licensee’s pet poses a risk to children in care.:
During the staff interviews, 3 out of 3 staff interviewed stated there are four dogs in facility; two dogs stay in an off-limits bedroom and the other two stay inside the off-limits garage. The Licensee stated licensee only takes the dogs out for bowel movement after they return from the park at around 10am, and at around 6pm after the children leave. 3 out of 3 staff interviewed stated dogs have all required immunizations. LPA inspected the off-limit garage and the off-limit bedrooms where the dogs are kept. LPA has conducted the inspections at the facility on 4/24/25, 8/6/25, 10/10/25, and 11/17/25, and observed the dogs being kept in the off-limit garage and the off-limit bedrooms.
During the inspection dated 11/17/2025, LPA observed the door leading to the garage has a plastic security door knob installed on the door handle making the garage inaccessible to the children. LPA also observed a baby gate installed on the off-limit bedroom door where the dog kept.
LPA interviewed 5 parents and 5 children. 5 out of 5 parents stated they are satisfied with facility.
