A plan of correction inspection was conducted at the facility by Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek. At 2:00 pm LPA met with licensee, Rasta Mahjan and her assistant, 's assistant, Ahmad Shah Rasta who were supervising 10 awake children of whom 4 were under 24 months old and 6 were over 2 years of age. Licensee was operating within the licensed capacity as specified on license for Large Family Childcare Home.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 11/14/2024 the facility was in violation of staffing ratio and capacity under section 102416.5(e) of California Title 22 Regulations by having 7 children of whom 4 were under 24 months old, 2 were over 2 years old, and one school age child without any assistant. A citation was issued on 11/14/2024. On 11/15/2024, the licensee sent a plan of correction to LPA stating they are aware of the staffing ratio and they would comply with it at all times. On today’s unannounced inspection, LPA learned that the facility has taken care of the issue of staffing ratio by having her assistant present to comply with Large Family Childcare Home. The deficiency is cleared now by this inspection. Children's files were reviewed and LPA observed that all the reports from previous inspection documenting type A citation were given to all the parents and a copy of LIC 9224 was in each child's file. No deficiency was cited today.
The licensee, Mahjan Rasta was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. The licensee, Mahjan Rasta was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The exit interview was conducted with licensee, Mahjan Rasta.
