An Informal Office Meeting was conducted on this day in the Orange Regional Office. Present during the meeting were Regional Manager (RM) Bertha Manzanares, Licensing Program Analyst (LPA) Gesine Connolly and licensee Mingqiong Zhang. This informal meeting was held with the assist of Language Link, interpreter 12475, as licensee Mingqiong Zhang’s primary language is Mandarin.

The purpose of the Informal meeting was to discuss the history of the licensed day care taking into account the deficiencies that have been cited. The RM reviewed the facility roster with licensee. Licensee stated that 8 children are currently enrolled for care and of this eight 4 are infants. The children's attendance was discussed and reviewed. Licensee states she has a contract with the parents specifying the time and date when children will be in care. Licensee also stated she has an assistant confirming that when operating as a licensed large family child care home the assistant is present.



The RM reviewed that in May 2017 a Non-Compliance Conference was held at which time the following was addressed: personal rights in that two infants were slept side by side in the same crib, criminal record clearances, physical plant in that the gate at the bottom of the staircase was not latched, roster, conduct inimical in that the licensee needs to be honest with the Department at all times and staffing and ratio as licensee was by herself was caring for 8 children 4 of whom infants.

The RM discussed that at this May 2017 non compliance conference the previous regional manager discussed the seriousness of the deficiencies and the importance of remaining in compliance.

Also discussed at the time of the Non-Compliance was:

Personal Rights: Each child shall be accorded safe, healthful and comfortable accommodations --- Infant in care was found by the LPA in a closet strapped in a highchair. Continued on page two

Today the above violation was discussed in relation to a similar issue:
California Title 22 Regulations 102417(a)
Operation of a Family Child Care Home Supervision 3/21/18 -- child climbed into a highchair without intervention and fell sustaining pain that required medical attention.

Licensee stated that her plan to prevent a reoccurrence of the misuse of a highchair is that when the highchair is not in use for feeding it is put in an area inaccessible by children in care. Licensee said she understands children are to be supervised at all times and provided safe and healthful accommodations at all times.

Licensee said she has hired an assistant who is with her on a daily basis Monday to Friday 9:00 AM to 5:00 PM. Licensee said when she takes a vacation the assistant takes the same time vacation. The licensee said if for any reason the assistant is not present it is her plan to notify parents as to who cannot attend the day care until the assistant is again present. Licensee operates with a large family child care license.

The following was discussed with the Licensee:
1. The licensee was advised that it is her responsibility to know & understand the requirements of Title 22 Regulations.
2. The licensee’s facility must be in compliance at all times.
3. The licensee was advised to check the Child Care Licensing web site at www.ccld.ca.gov for quarterly updates, forms and regulations.
4. Licensee will be put on required visits for two years.

RM Manzanares explained If subsequent repeated violations are cited in the future and the Department determines that the facility has violated the law/regulations or is inadequately implementing the approved plans to remedy the facility's noncompliance, the Department, in its discretion, will seek formal legal action.

Exit interview conducted with licensee. RM explained to licensee that a copy of this office visit report would need to be given to parents of all children in care and the Acknowledgement form would need to be signed and placed in each child's file. A copy of report would need to be provided to all new parents for the next twelve months.
