An Annual Random inspection was conducted at the facility by Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek. The Covid-19 Emergency Response questionnaire was reviewed and answered by the co-licensee, Natalie Valdivia before entering the facility. The other co-licensee, Padmini Srinivasan was not present. LPA observed 8 children of whom one child was under 2 years of age with co-licensee's assistant. The facility was operating within the licensed capacity as specified on license. Due to State of Emergency COVID-19, LPA observed the facility was following CDC and Dept of Public Health Guidelines.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently there are two adults including the co-licensee and no minor children living in the facility. Facility Day care hours are 7:00 am-5:30 pm, Monday through Friday.
During today’s inspection, LPA and co-licensee toured the inside and outside areas identified in the facility sketch as accessible to childcare children. Off limits areas are made inaccessible by means of baby gates and locked doors. The childcare area consists of family room, dining area, living room, children's bathroom, and outdoor patio with backyard. Main childcare room is accessed upon entering the home. LPA observed a wooden slide and a wooden monkey bar in part of the living room set up for children with plastic mats underneath. There are working carbon monoxide, smoke detector, and fire extinguishers in the home that meet statutory and State Fire Marshall standards. Detergents, cleaning compounds, medicines, and other items which could pose a danger to children were not readily available to children they were stored inaccessible to children, none were observed during inspection. Licensee stated there is no firearm in the facility.

receive the updates. The home has age appropriate toys for the ages served. LPA verified there is a working telephone service (cellular service). It was stated, they use the backyard for outdoor play. The outdoor area was fenced. There is no body of water observed.
There is a current roster of children in care. Children’s records for children present during LPA’s inspection were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700) and found to be in compliance.
The co-licensee's Pediatric CPR/First Aid certification and her assistant's cards are current. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family childcare home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for co-licensee and her assistant were reviewed and were within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training, and to renew the training every two years. Co-licensee and her assistants' mandated reporter training certificates were not available.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The licensee understands she must be present in the facility and must ensure children in care are always supervised. Children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care and supervise children while absent. The substitute adult must have the required criminal record, child abuse index clearances, immunizations, Pediatric CPR/First Aid, and mandated reporter training. The licensee's assistant met all the requirements. Licensee was advised of their responsibility to review the Provider Information Notices (PIN) found on the CCLD website. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the licensee. LPA reviewed with licensee the following safe sleep best practices:
· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used if they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold.

In the areas that were evaluated, the following deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. The deficiencies are cited on LIC 809D.
An exit interview was conducted with licensee. Appeal Rights were explained. The Licensee was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. The Notice of Site Visit was posted and discussed as required by H&S Code Sec. 1596.817. Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00. The Notice of Site Visit must be posted on or adjacent to the door. LPA informed the licensee to send an updated LIC 279 to our office.

On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs(2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years

This requirement was not met as evidenced by LPA reviewing two staffs' files today at 12:15 pm finding no staff had certificates. The facility failed to meet this requirement of Title 22 Regulations. This is a potential risk to the health and safety of children in care.

The licensee shall document the drills, including the date and time of each drill. This documentation shall be kept at the family child care home. This requirement was not met as evidenced by the licensee's admission at 12:15 pm that she had not done the fire drill with children for a long time.
