Type ‘B’ deficiency: (h) (1) "Home" means the licensee's residence as defined by Government Code
Section 244.
On 5/4/2022, Licensee and Licensee’s husband both admitted to living at a different residence since September 2021, stating that it was hard to live where they worked and wanted to live somewhere separate. Licensee and licensee’s husband admitted to living at a different location, declarations were obtained with their home address. (Substantiated Complaint)
1596.80 Child day care facilities, licenses
Type ‘B’ deficiency: No person, firm, partnership, association, or corporation shall operate, establish, manage, conduct, or maintain a child day care facility in this state without a current valid license, therefore as provided in this act.
On 3/5/2021, It was stated by several parents that for at least 2 months, the spouse/assistant of licensee provided care to other children at a nearby home. (Substantiated Complaint).
The department received a new application for spouse of licensee at a new location. Vladislav stated he is in application process. Per Vladislav, Licensee Yulia and spouse Vladislav are living in different households currently. LPM Magana advised Licensee to update lease agreement stating all adults/tenants need to be fingerprint cleared, and associated before initial presence in the home. LPM Magana advised Licensee to not have guests in the home during facility operating hours. LPM Magana advised Licensee and Licensee's spouse to remain in constant communication with the Licensing Department. LPA Gontarek advised Licensee to utilize Child Care Careers to access substitute teachers. LPA Gontarek signed Licensee and Licensee's spouse up for Provider Information Notices (PINs) on our website: www.cdss.ca.gov/inforesources/community-care-licensing to receive important updates. LPA advised Licensee to review Guardian video tutorials, and use Guardian to associate adults to the license. LPM Magana advised Licensee and Licensee's spouse to attend Licensing Calls/Webinars.
LPA/LPM explained and provided the TSP Brochure. Licensee agreed to be referred for Technical Support Assistance (TSP).
Licensee will provide a copy of this report and LIC 9224 (Acknowledgement form). |
