An Informal Office Meeting was conducted on this day in the Orange Regional Office. Present during the meeting were Licensing Program Manager (LPM) Rina Lopez, Licensing Program Manager (LPM) Patricia Magana, Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek, and Licensing Program Analyst (LPA) Alana Gontarek. Also present in this meeting were, Licensee, Tatiana Miranovich facility # 304313906 located at 45 Langford Ln, Ladera Ranch, CA 92694, Licensee, Alevtina Lopatin facility # 304313282 located at 14 Japonica, Irvine, CA 92618, and Alexey Lopatin. The office contacted the Translation Agency over the phone for interpretation the meeting for the above attendees whose native language were not English.

The purpose of this informal meeting is to discuss the licensee's most recent inspections and the request of licensee, Simon Lopatin to add Alexey Lopatin as co licensee temporarily while the Licensee, Simon Lopatin is not present due to unforeseen circumstances.

On 2/17/2022 Department received a complaint alleged licensee, Simon Lopatin is not present in the day-care facility. Based on the interviews with staff working at the facility, parents, and the Licensee's admission being out of United States, the complaint allegation was substantiated. The Licensee was issued the citation under Section 102417(a) under Operation of a Family Child Care Home on 2/25/2022.

The licensee shall be present in the home and shall ensure children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.

According to the Licensee, Simon Lopatin, he is still out of United States. Simon has requested the Department to add Alexey Lopatin to the license as a co-licensee temporarily until he returns to United States.

On 4/4/2022 Licensee, Simon Lopatin submitted the following documents to add a co-licensee for Alexey Lopatin:

LIC 279 signed by both licensee and co-licensee as an update application.
LIC 999 (sketch of the house)
LIC 508 (criminal record statement)
Fingerprinting clearance and association to current license.
Certificate of orientation for Family childcare home
Certificate of Mandated Reporter Training
CPR and 1^st aid and Health and Safety Training Certificate
TB test clearance, MMR, Pertussis vaccine. Flu shot is optional.
One year experience working in a childcare facility or being a family childcare provider.

During the inspection of 2/22/2022 to Simon Lopatin’s facility located at 110 Prairie Rose, Irvine, CA, Licensee, Tatiana Miranovich facility # 304313906 who has a Family Child Care Home located at 45 Langford Ln, Ladera Ranch, CA 92694 arrived at this facility within 45 minutes of LPA's arrival after a staff person notified Tatiana of LPA’s present at this facility. Tatiana discussed the operation of the facility and the allegation of the complaint regarding Simon is not present at the facility with LPA. Tatiana was present during LPA’s present for or about 2 hours. Also, LPA was advised of the presence of Licensee, Alevtina Lopatin # 304313282 who has a facility located at 14 Japonica, Irvine, CA 92618 at this facility. Per interviews conducted, Alevtina provides homemade food for the facility every morning.

LPM Lopez discussed the recent issues with the attendees. LPM explained the presence and the role and responsibility of each Licensee for the location they reside at as a family childcare home.
LPM Lopez explained the responsibility of a co-licensee and his/her involvement and presence in childcare home and children's activities to the potential co-licensee, Alexey Lopatin. He confirmed that he understood
all the regulations and is familiar with it. Alexey Lopatin stated he moved in to the above address on 4/4/2022. He stated he cleans the facility, he also cooks, takes care of children if he was asked to by one of the assistants. He stated there are other assistants who work at the facility as well.

LPM Lopez asked Alexey Lopatin if he understood the family childcare home orientation online? He confirmed he did. LPM confirmed with Alexey Lopatin that he needed to reside at the above facility as long as he is the co-licensee. He confirmed he is aware of it.

The licensee, Simon Lopatin joined the meeting via phone. LPM requested a written plan for the issue that the licensee is not present at the facility. A plan will be submitted to our Department by next Friday 5/13/2022 that how this issue will be taken care in long term. Simon Lopatin will let our Department know if additional time is needed.

Alexey Lopatin is advised to review the Department's Child Care Video Series at:https://ccld.childcarevideos.org/. for awareness and familiarity of regulations as a co-licensee; some of the topics include but are not limited to reporting requirements, criminal record clearance, locks and inaccessibility requirements in childcare facilities. The licensee will check the Department's website www.ccld.ca.gov for any updates on a quarterly basis and will ensure compliance with California Code Title 22 Regulations.

Upon receipt of this report pertaining to a conference conducted by a local Licensing Agency in which issues were discussed, the licensee must provide a copy of this report to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months. Obtain signature and date from the child's parent/guardian on the Acknowledgement of Receipt of Licensing Reports (LIC 9224). Keep a signed LIC 9224 acknowledging receipt of this report in the child's file.

A copy of LIC 9224 was given to the licensee on today's meeting.

A copy of this report will be sent to Resource and Referral Agency (Children's Home Society of California) in Orange County. A copy of the Appeal Rights was given to Alexey Lopatin on this Office Meeting.

The Department will make more frequent inspections of this facility for compliance. The licensees were advised that it is their responsibility to know & understand Title 22 Regulations. The facility must comply with licensing regulations at all times.
