Licensing Program Analyst (LPA) Corral conducted an announced Pre-licensing Tele-Inspection of the facility on today's date with the Applicant Nirosha Silva using FaceTime.
A review of the Facility Personnel Report Summary conducted on 05/01/2020 indicates all facility residents, staff, or other individuals who require criminal background check clearances or exemptions. The residents in the home are the Applicant, Nirosha Silva and her 2 minor children. Applicant and her minor child were present during the Tele-Inspection.
The facility is a single-story home with 4 bedrooms, 2 bathrooms, a kitchen, 2 dining rooms, and living room. The rooms accessible to children in care are the living room, both dining rooms which were turned into 2 separate children rooms (toddler room and lunchroom), the kitchen, the bathroom down the hallway, and the first bedroom on the right down the hallway which will be used for infants. The backyard is also accessible by 2 different sliding doors one located in the lunchroom and one in the toddler room. The backyard has cement and grass area, there was also a fountain behind a fence with water less than 1-foot high. Applicant was advised outside play requires supervision at all times. The off-limit areas of the facility are 3 bedrooms that are located down the hallway which include the second bedroom down the hallway to the right, the master bedroom down the hallway to the left, and the second bathroom next to the master bedroom. The third bedroom that is off limits is across the infant room. All rooms will be inaccessible by lock and key. Applicant is aware that off limits areas must be completely inaccessible during hours of operation.
During the kitchen Tele-Inspection, the knives were located in a top kitchen shelf above the oven, 5 feet high. The facility does have a fireplace which was covered by a cubicle organizer that was securely mounted and tested by the applicant.
