On 06/10/2025, a complaint was filed with the Department allegation are (1) Staff handled day care child in care in a rough manner. (2) Staff caused injury to day care child in care. During the investigation, LPA Trinh conducted a physical plant inspection and interviewed four (4) staff members and five (5) children, and three (5) parents. LPA Trinh also obtained pertinent documents.

Regarding allegation (1), Staff handled the daycare child in care in a rough manner. During the staff interviews, four (4) interviewed staff members described how to provide positive discipline for the children in care, including positive giving redirections, offering choices, and encouraging the children to make positive choices. However, Staff #2 (S2) stated in the interview and provided a declaration that while trying to prevent Child 5 (C5) from hitting other children in the classroom, S2 became physically involved and was rough in their maneuver, which caused the child to get hurt. LPA interviewed five (5) children. During the interviews, C5 provided a statement regarding getting hurt by S2 and pointed out to the LPA the location where and how S2 hurt C5.

During record review, Facility Handbook Policy’s guidance strategies states, “Positive guidance and discipline are crucial because they promote children’s self-control, teach children responsibility, and help children make thoughtful choices”. Based on this policy and statement provided by S2, S2 did not follow the facility’s policy regarding guidance strategies.

Regarding allegation (2) Staff caused injury to day care child in care. All four (4) interviewed staff members stated that there was a written incident report which revealed that C5 was sustained a bruised elbow and redness on the forehead during the incident.

On 7/25/2025, LPA called five (5) parents requesting an interview. The parents interviewed did not express any concerns related to the allegations and did not provide any information that could corroborate the allegations.
