On 7/31/2025, the Orange County Child Care Office received a complaint with allegations: (1) Provider does not follow infant safe sleep practices, and (2) Provider does not provide adequate supervision to napping infants.
During the investigation, LPA conducted facility visits on 8/4/2025. LPA Trinh interviewed seven (7) staff members and four (4) parents. LPA Trinh also reviewed records, which included the LIC 500 Personnel Report, LIC 9040 Children’s Roster, the Personnel Handbook, the Infant and Toddler Safe Sleep Check, and the Staff time sheet from July 14, 2025, to July 25, 2025.
Regarding the allegation (1) Provider does not follow infant safe sleep practice. During visits, 8/4/2025, the LPA observed two (2) infants being held by one (1) staff member in the floor playroom area, and four (4) infants who were still not sleeping inside the swing sleeping area with two (2) staff members. When the LPA received the completed tour inspection, the LPA returned to the infant classroom and observed the teacher beginning to transfer one (1) infant from the swing to the crib to sleep. During today's inspection, 9/22/2025, LPA witnessed the infants sleeping in the cribs with the teachers in the napping room, and staff followed safe sleep guidelines. During interviews, all seven (7) staff members stated that when children fall asleep, they move them into the crib and conduct a 15-minute sleep check. Additionally, all seven (7) staff members indicated that they received training from the director on safe sleep regulations for infants and understood how to ensure infant safety. The four (4) interviewed parents did not disclose any information related to the allegation.
Regarding allegation (2) Provider does not provide adequate supervision to napping infants. During the visit on 8/4/25. LPA observed three (3) fully qualified infant staff members and ten (10) infant children during the transition from lunch to nap time. During today's visit, 9/22/2025, the LPA observed four (4) fully qualified staff members and 14 infant children in the nap room. Based on the interview with seven (7) staff members, they all stated that there is always an adequate ratio of infant staff members in the classroom while children are napping. In addition, all seven (7) staff members indicated that staff need to complete the 15-minute checklist for each child in the classroom. According to the records reviewed, all infants in the school receive a safe sleep check every 15 minutes. The teacher time sheet review from August 1, 2025, to August 25, 2025, indicates that in the infant/toddler classroom, there are always 6-8 staff members from 11:00 a.m. to 2:00 p.m., following a rotating lunch schedule
