32 | Reporting Party (RP) stated during pick up, a child was in the restroom alone crying having had an accident and S1 was seated in the adjacent room on their mobile device making no attempt to assist the child. RP stated, children sometimes do not have their diaper changed until after nap time.
During the course of investigation, LPA toured that facility, reviewed facility’s Parent Handbook and obtained facility roster. LPA conducted interviews with 4 staff members, 4 children, 4 parents and reviewed records.
Regarding allegation (1) Staff left day care children unattended.
LPA interviewed 4 staff members. 4 out of 4 staff members stated they have not witnessed staff leave day care children unattended. Staff # 2 (S2) stated, on one occasion Child #1 (C1) was crying due to a bathroom accident. S2 stated, that occurred a while ago; however, C1 was not left alone crying in the restroom. Staff #3 (S3) stated, C1 had a bathroom accident once, and they were whimpering and crying, but staff all always present to provide support, help the child use the restroom or change into clean clothes. S3 stated, C1 was not left alone.
Regarding allegation (2) Staff are not meeting day care children's diapering needs
LPA interviewed 4 staff members. 4 out of 4 staff members stated they have not witnessed staff not meeting day care children's diapering needs. Staff #2 (S2) stated that the policy is to change child when needed, if they are wet or soiled, and staff will change child. Staff will check during each transition, especially before and after nap. Staff #3 (S3) stated, staff document on bright wheel to let the parents know child is changed, staff will check the children throughout the day, before nap time, after nap time and, after snack, every transition staff will check them and change them if they need to be changed. 4 out of 4 staff disclosed that Diaper changes are documented on Brightwheel app. LPA obtained Brightwheel diaper log for the month of July 2025 and reviewed log. LPA observed log documenting diaper change approximately every 2 hours to 2.5 hours. Diaper log stated date, child's first and last name, Room, time and diaper status whether diaper was wet or dry.
LPA Duron interviewed 4 children. The 4 children interviewed made no disclosures regarding the above allegations.
LPA Duron interviewed 4 parents. All interviewed parents stated they did not have any concern with facility.
