On 09/12/2023, Licensing Program Analyst (LPA), Anna Chan conducted an onsite inspection for the purpose of an unannounced Annual Visit. LPA met with Team Lead Kaithlyn Smith and toured the facility inside and outside and the floor and yard plan were verified. Director Kelsie Nichols arrived the facility about 15 minutes later. The facility is located in the same parking lot of 24hour Fitness gym in Anaheim Hills Festival. There are no children at the time of visit at 8:45am. This is a school-age after school program. This program serves ages 5 through 12 years old.

It was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30a.m.- 6:00p.m., Monday through Fridays.

A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; PM snacks are provided. Wednesday lunches are provided when school has short days. Food prep areas were clean and sanitary. Food is properly stored; Menus were posted near the entrance walkway to the office and at the kitchen area. Copy of Menus are sent to parents on Friday's for the following week. Allergy list of children are also posted in the kitchen and in the classrooms.

Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Currently, only one classroom (classroom on the left upon facility entrance) is being utilized by the program since there is a low enrollment (18 children). LPA advised facility to make sure that the other classroom located on the right of the entrance is well maintained, sanitary and in good condition before re-opening. There is a purified drinking water available to children indoors with disposable cups and children have their own sports bottles properly labeled with their names on it.

There are 2 bathrooms inside the classroom. Team Lead stated that it is being cleaned by the cleaning crew every night or before the children arrives the facility. The facility has conducted an emergency drill within the past six months on August 15, 2023. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility

Staff files were reviewed for staff present during the facility inspection. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employees or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for the staff were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 01/2024.

Children's records were reviewed, and there was a separate, complete, and current record for each child. In the areas reviewed, 5 children’s files were found to be in full compliance. Sign in/out was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day. Children are being dropped off at the facility between 6:30am and 7:30am. The facility offers transportation for after school children at Running Springs Elementary.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services (IMS) LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The outdoor activity space was inspected for compliance. The cushioning is a black top cushioning around the climbing equipment, such as slide and other similar equipment appeared to be enough to absorbs falls. The other side of the playground has a turf grass and a basketball court. Drinking water in the outdoor activity space is provided by purified water gallon and disposable cups. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility ground was safe, sanitary and in good repair.

Information on the additional nutrition training, immunization requirements for children, and Health Schools Act (http://www.cdpr.ca.gov/docs/pestmgt/schoolipm.htm) were provided. The Site Supervisor was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative.

In the areas that were evaluated, NO deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

An Inspection and exit interview were completed with Director, Kelsie Nichols. The report was reviewed and discussed. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
