During record reviews, LPA Tran reviewed facility's Family Handbook as well as the myPath app, which was used by the staff to address and manage challenging behaviors including bullying, it showed that the facility use Positive Behavioral Support model to prevent and respond when any child exhibits challenging behaviors. These strategies can be used to help child acquire skill for self control, set limits and making appropriate choices. LPA Tran also review facility's documentation on steps taken to address the concerns of the alleged incident. It showed that Director took immediate action to interview the children and communicate with parents, communicate with team members to be vigilant when providing supervision for the children, and keep safe of child's belongings.
Based on the information gathered from LPA's interviews conducted with 6 staffs and 7 children on 05/20/2022 and on 08/03/2022, and record reviews of staff training, facility’s Family Handbook and action plan taken by Director to address the concern of bullying behaviors, there is insufficient evidence to corroborate the allegation that Day care child is being bullied by other day care children, therefore, the allegation that Day care child is being bullied by other day care children is unsubstantiated.
Exit interview was conducted. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. “The licensee was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights.” This report LIC 9099 was provided to the director Perla Arrieta. First level appeal is to Regional Manager, address is above on the report.
