Licensing Program Analyst (LPA), Yesenia Villa conducted an onsite inspection for the purpose of an Annual Random. LPA toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. LPA Villa did a walk thru of the facility at approximately 12:30pm with Ms.Casie and Ms. Dani. All staff were observed wearing masks and following COVID-19 protocols.

Census was taken in three rooms, the 3 yr. old room had 14 children present with two staff, the 2yr. old room had 10 children present with 2 staff and the 4yr, old classroom had 14 children present with three staff. The 14 children from the 4yr. old classroom was outside at an off-limit restroom on location at Grace Christian Academy. Per Ms.Casie there was a waiver issued for the use of the facility over 10yrs. ago. LPA Villa will verify the waiver is on file.

During this inspection it was observed that the facility was observed to be within ratio. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. This facility operates Monday to Friday from 6:00 am-6:00pm. Ages served are 2-4yr olds.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. As per Director parents provide snacks and lunches, there was accessible water for the children indoors by means of labeled water cups and water fountains. All food was stored properly and observed to be labeled. Menus were posted where they could be reviewed by parents.

Children nap on cots/mats, and bedding is stored individually and is taken home weekly to be washed by parents as stated by Ms. Dani. The facility has conducted an emergency drill within the past six months. The facility has a working carbon monoxide detector and fire extinguisher it is serviced by Cypress Unified School District. Facility met all posting requirement. Staff files were reviewed for staff present during the facility inspection this date.

Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for five staff were reviewed and within compliance. (Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 02/27/23. Five out of five staff were found to be in compliance.

Eight Children's records were reviewed, and there was a separate, complete and current record for each child. In the areas reviewed the children’s files were found to be in full compliance. Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day.

This facility provides Incidental Medical Services - (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at
least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The
cushioning material used under the apparatus is sand. Per Ms. Dani Cypress School District refills the sand as requested usually twice a year. The children have access to a water fountain in the preschools section of the playground. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair.

Covid-19 guidelines were discussed with the Licensee and the Assistant Director. This face covering guidance is for all individuals 2 years and older, except for the exceptions (child’s development, medical exemptions, etc.) that are outlined by CDPH.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.



An exit interview was completed with the licensee Ms.Casie and Assistant Director Ms. Dani.

The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days.

The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
