S1 stated on 03/15/2024 they took 7 children for dismissal alone, S1 was at the front of the line guiding the children through the hall which is about 70 feet from the preschool classroom. S1 stated after walking 40 feet down the hallway they turned around to check on the children and S1 observed that C1 was not in line. S1 stated they immediately called S2 to ask if C1 had returned to the classroom. S2 confirmed C1 was inside the classroom, S1 asked S2 to bring C1 outside to rejoin with S1’s group. S2 waited outside the classroom door while C1 walked 50 feet alone while C1 rejoined S1’s group. S1 stated they take responsibility in not conducting a head count before they left the classroom. All the staff disclosed since the incident S4 told them that 2 staff need to escort the children during dismissal.
During the investigation on 03/20/2024 LPA interview 6 children. C1 disclosed they were crying the other day because they were left outside the classroom alone. None of the children recall the incident with C1. All the children disclosed that the staff are always supervising them.
Based on LPA’s facility inspection, observations, interviews conducted with 4 staff, 6 children and records reviewed, it has been determined that S1 was not supervising C1 for about 1-2 minutes during dismissal. Therefore, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, Title 22, 101229(a) Responsibility for Providing Care and Supervision is being cited on the attached LIC 9099D.
LPA Odom informed Early Learning Specialist Laura Barnette that this report dated 04/09/2024 document 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Odom informed the Early Learning Specialist Laura Barnette to provide a copy of this licensing report dated 04/09/2024 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
