25 | A case management follow up inspection was conducted at this facility by Licensing Program Analyst (LPA) Hawkins. During todays inspection LPA toured the facility inside and outside and observed a total census of 15 preschool children with 3 preschool staff (room 506). There were no children/staff present in room 404 (AM/PM schedule) upon arrival. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
LPA met with lead teacher Angelina Cortes and discussed the unusual incident report in which preschool staff was accused of violating a child's personal rights on 10/22/19. The allege incident was reported to Licensing on 11/7/19. During today's visit LPA interviewed 11 children in Room 506 (which was the identified class the alleged incident took place in). After reviewing records, interviewing 4 preschool staff, 11 preschool children, and several attempts were made to interview parent involved, LPA was unable to determine if the child's personal rights were violated by preschool staff.
The facility is within compliance during today's inspection.
Exit interview was conducted with lead teacher. Notice of Site Visit was posted during the visit. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The representative was provided a copy of their appeal right (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. This report is to be on file and accessible for public review at the facility for at least 3 years. |
