Licensing Program Analyst (LPA), Cynthia Sun conducted a visit for the purpose of a Plan of Correction (POC). LPA was given a tour of the preschool classroom inside and outside by Early Learning Specialist, Laura Barnett. Census was taken as follows: 3 staff supervising 11 preschool children from room #506. All children were supervised while preparing for music and then transitioning for breakfast.

A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

On 01/31/2025 LPA conducted an unannounced case management inspection in response to a self-report Unusual Incident dated 1/22/2025. LPA observed play structure equipment where C1 fell, was labeled “this equipment has been designed for users between the ages of 5 to 12 years old”. The facility is licensed to care for 48 children ages 2 through 5 years old. Based on LPA’s observation, the play structure is not age appropriate for children in care at the facility. The children access to play structure poses an immediate health, safety, or personal rights risk to persons in care. A Type A citation was given according to California Code of Regulations, Title 22, Division 12, Chapter 1, Sections101239 Fixtures, Furniture, Equipment and Supplies (m) All play equipment and materials used by children shall be age appropriate.

On 02/03/2025 In the areas that were evaluated, the plan of correction was in compliance.

LPA observed that the children's playground play structure was blocked off with orange cones and caution yellow tape to ensure children did not use play structure. Early Learning Specialist stated they will speak and teach children to play in approved areas. Early Learning Specialist spoke to parents to remind their children to not play in play structure because it’s not safe for their age and spoke to facility custodian and the principal to help with blocking the play structure on a daily basis.

In the areas that were evaluated, NO deficiencies were observed in the California Code of Regulations, Title 22, Division 12 at the time of the visit.

Exit interview conducted and report was reviewed with the Early Learning Specialist, Laura Barnett. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Appeal Rights were explained. The Licensee was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above.
