An unannounced follow up complaint inspection was made on this day by Licensing Program Analyst (LPA) Alanna Gontarek to the licensed day care center. LPA met with Assistant Director, Pamela Mammen for the purpose of delivering findings of the complaint received 11/2/2021. There were a total of 10 staff and 47 children present during this inspection.

Per complainant, allegation states, “Over the past 2 months, complainant picked up complainant’s 3-year old child, and child had wet diapers on multiple occasions.” No specific dates were given.

During investigation, LPA conducted interviews with staff and children’s authorized representatives. During visit to facility on 11/08/2021, LPA made observations, and gathered the following documentation which includes: Facility Roster, diaper charts, ‘Tad Pole’ application messages from children age 3’s children’s authorized representatives, and Director’s email to Licensing Department.

Pertaining to the allegation: Day care children are being left in soiled diapers, on 11/8/2021, during LPA interviews conducted with staff, multiple disclosures were made stating children’s diapers are changed as children are waking up after their nap. Per staff interviewed, corroborating statements were made that diaper checks are completed throughout the day, at specific times, and on a as needed basis. Multiple staff disclosures were made also stating that if a child’s diaper visually looks full, is sagging, or the child tells a teacher they need to be changed, a staff member will take the child to get diaper changed. Per children’s authorized representatives interviewed, multiple disclosures were made stating there are no issues regarding their child’s diapers at pick-up time, and reported no other concerns. Per staff and parents interviewed, corroborating statements were made stating the school checks and changes the children’s diapers every 2 hours. Staff #2 (S2) stated, during interview conducted with LPA, staff were not always logging diaper changes consistently, but staff changed the diapers as needed. During record review, LPA observed Diaper charts (for the children in diapers: C1, C2, C3) showing diaper changes completed between 3:00 p.m. and 4:00 p.m.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Exit interview conducted with Assistant Director, Pamela Mammen. A copy of this report and Appeal Rights were provided.

A "Notice of Site Visit" and copy of the report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty.
