Licensing Program Analyst (LPA), Yesenia Villa conducted an onsite inspection for the purpose of an Annual Random. LPA toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. Census was taken in the infant room LPA Villa did a walk thru of the facility at approximately 8:50am. There was a total of 7 infants present upon arrival. LPA Villa observed (6) infants/walkers with two staff in the first partition of the infant room, there was (1) napping infant in the second part of the room with (1) staff. The facility was observed to be within ratio. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children.

Poisons/Hazardous Items are not kept on the premises. As per Director parents provide infant snacks and lunches, infants also bring in their own water cups. All food was stored properly and observed to be labeled.

Menus were posted where they could be reviewed by parents. LPA Villa observed the changing tables in the infant room with debris (pictures were taken), Licensee was able to clean the changing tables during the inspection. Floors and furniture were clean and were observed to be in good repair and free of sharp edges.

There is drinking water available to children indoors and outdoors by sports bottle with the child’s name on it. Children nap on cots/mats, and bedding is stored individually and is taken home daily to be washed by parents. The facility has conducted an emergency drill within the past six months. The facility has a working carbon monoxide detector and fire extinguisher that is serviced by the Cypress School District. Facility met all posting requirement.

4 out of 4 staff files were reviewed. 1 of out 4 staff did not have immunization on file, however the staffs first day was 08/09/21. Director will get documents to licensing within the week. Health screening and immunization's as required were reviewed for all staff. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for (licensee and assistant) were reviewed and within compliance. (Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 02/27/23. Children's records were reviewed, and there was a separate, complete and current record for each child. In the areas reviewed the children’s files were found to be in full compliance.

Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day.

This facility provides Incidental Medical Services - (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material used in the outdoor infant playyard is grass. Per assistant director there are mats used for extra cushioning material for the infants.
LPA Villa provided Safe Sleep resources and print out to the Director during this inspection.

· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold

Updated face covering guidance was provided and Assistant Director and Licensee Casie Hollenbeck was to continue to message the “Three W’s”: Wash your hands. Watch your physical distance. Wear a mask. This face covering guidance is for all individuals 2 years and older, except for the exceptions (child’s development, medical exemptions, etc.) that are outlined by CDPH.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

An exit interview was completed with Casie Hollenbeck and Assistant Director Ms. Danny. The report was reviewed and discussed. Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
