1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Page 1 of 4 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek and LPA Mila Quinto conducted a case management inspection of the facility for the purpose of decreasing their capacity from 142 to 98 school age children by removing rooms 105, 106, 107, and 108. Rooms 105, 106, 107 will be added to preschool program and room 108 belongs to children from Department of Education with private school affidavit. LPAs met with director, Chunyu Hsu who gave LPAs a tour of the facility. The facility is located on the Chinese Cultural Community on the first floor. They are currently licensed for 142 school age children in rooms 105, 106, 107, 108, 109, 110, 111, and 134. There are no children enrolled for the school age program. The applicant would like to keep the school age license active for future enrollment. The facility has requested to provide care and supervision to 98 school age children ages 5 to 13 years old, Monday to Friday from 8:00 am to 7:00 pm. Parents will sign in and out their children manually in the lobby in front of the entrance of the building. The community Center is a two story building which the school age program is located on the first floor. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. All areas identified on the Facility Sketch were inspected. If children become ill during the day, they will rest in the director's office, and use the adult restroom if needed until sick children are picked up. Facility met all the licensing posting requirements. Following was observed in all the rooms: Age appropriate furniture and activities including educational, arts & crafts items. Rooms are well ventilated with proper lighting and shade. Continued on page 2

There is carbon monoxide in the hallway.

The measurement of the rooms according to updated numbers are as below:
Room 109 is: 520.60 square feet
Room 110 is: 500.40 square feet
Room 111 is: 242.97 square feet
Room 134 is: 235.73 square feet
Total indoor measurement for the rooms are 1499.70 square feet divided by 35 square feet = 42.85
(43 children)

Total toilets: 3 X 15= 45 children
Total sinks: 3 X 15 = 45 children.
The school age children is using the Boys' bathroom with 3 toilets and 3 sinks with the sign stating "children's restroom". The restroom is equipped with 4 other stalls, 7 urinals and 4 other sinks that will be shared with school age children. Room 108 with private school affidavit will share the restroom with school age children of this program. The director was reminded that children shall be escorted to and from the restrooms by an adult. A waiver will be requested to share the restroom area since there are other stalls in this restroom additionally.

Outdoor Activity Space:
Total measurement of the outdoor space is: 7749.38 square feet divided by 75 square feet=103.33
(103 children). The facility has currently a waiver on file to share the playground with school age children on a rotating basis.
Outdoor spaced is completely enclosed. Outdoor activity area is supplied with age and size appropriate equipment with adequate of cushioning materials under climbing structure and slide. Shade is provided by trees, and awning. There are two exit gate on the playground leading to parking lot. One is always locked

and the other one is opening by a push bar. The director stated the push bar gate cannot be locked according to Fire Marshal. Director stated one teacher is stationed in front of the push bar gate and another teacher on the other side leading to the other gate for better supervision of children when they are outdoor.

LPA gave a copy of the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. The director's Immunization, Mandated Reporter Training, Health and Safety Training are current. Her CPR and 1st aid is EMSA approved and expires 1//2025.

The director, Chunyu Hsu reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Exit interview conducted and report was reviewed with the director, Chunyu Hsu.

Fire clearance approval has been received in our office to approve the capacity of 98 children. However the measurement of the classrooms of 109, 110, 111, and 134 are accommodating 43 school age children.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Based on today’s measurements, the sinks and toilets availability, the request of the facility for decreasing the capacity to 43 school age children in rooms 109, 110, 111, and 134 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required.

1- Waiver request to share the restroom area for school age children and others. For waiver please send a waiver request letter, LIC 999, LIC 500 and a photo of the restrooms to display the signs for children's use.
2- A copy of private school affidavit for room 108.

Updated LIC 500, LIC 308, and LIC 610, and LIC 309 were submitted to LPAs on today's date.
