During the course of the investigation, LPA Corral conducted 8 interviews with 7 Staff Members and Director Ashley Neitzke regarding the above allegation. During LPA Corral interviews with the Staff Members, Teachers were asked if children outside of the infant age group were observed to be in the infant classroom, all 7 teachers including the Director stated No. LPA Corral also asked if other children outside of the infant age group interact or play with infants, all Staff Members interviewed stated No. LPA Corral wanted to confirm that the only children allowed in the infant classroom are the infants, both infant teachers stated Yes. One preschool teacher stated she doesn't put her children in the infant room and infant teachers don't put babies in her preschool classroom. Both infant teachers stated the only children allowed in the infant classroom are the infants. During the Investigation, LPA Corral also requested the sign in/sign out sheets for the infant program, and the preschool program. During the Record Review LPA Corral confirmed that the children signed in the infant classroom are in the infant age group. The Children’s Roster was also reviewed, infants Date of Birth were reviewed and verified.
LPA Corral conducted 8 Interviews which included the Facility Director, two Infant Teachers and five Preschool Teachers. Record Review was also completed to investigate the allegation, based on the information obtained there is not enough evidence to substantiate the allegation stating Staff Commingle Day Care Children therefore, the allegation is found to be Unsubstantiated. This agency has investigated the complaint alleging that Staff Commingle Day Care Children, although the allegation may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
LPA Corral conducted an Exit Interview with Director Ashley Neitzke. The report was read, reviewed and discussed with Director. Appeal Rights were explained, Director was provided a copy of her Appeal Rights and signature on this form acknowledges receipt of these rights. Director was informed all appeals must be in writing and received by the Regional Office within 15 business days. Notice of Site Visit was provided and posted by Director. Director was informed the Notice of Site Visit must be posted for 30 consecutive days and failure to post will result in a Civil Penalty of $100.00.
