25 | On 2/28/2024, Licensing Program Analyst (LPA) A. Silva conducted a Case Management – Deficiencies due to deficiencies observed. An on-site Facility Personnel Report Summary review indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Census was 90 preschool children at the time of visit. The facility was operating within ratios and capacity.
The licensee did not have a current roster of children in care. Per Ch 3.4 California Child Day Care Act, Health and Safety Code 1596.841 Current roster of children provided care in facility required, “Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.” The director was provided a copy of the LIC9040 Child Care Facility Roster.
Based on records review and interviews with Director Abeyta, the facility is being cited in accordance with the California Code of Regulations, following section: HSC 1596.841 Current roster of children provided (See LIC809D attached).
Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with director Ruby Porter. |
