On 06/17/2020, LPA Nguyen contacted six parents by phone and was able to talk to two parents. The interviewed parents stated they like the day-care and all staff members are very nice. One parent stated the school informed all parents about head lice regardless of their children having head lice or not. Interviewed parents also stated they did not have any concern or issue with the facility.

Based on the information gathered from LPAs' interviews, there is insufficient evidence to corroborate the allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged facility staff are not addressing bullying occurring between children in care, facility staff yelled at children in care, facility staff failed to prevent the spread of lice, and day care child hit another child in care violations did or did not occur, therefore the allegations are unsubstantiated.

Exit interview was conducted.

The Notice of Site Visit was not posted due to tele-investigation Covide-19 State of Emergency. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) will be provided through email and their signatures on this form acknowledges receipt of these rights.
