Licensing Program Analyst (LPA), Mila Quinto conducted an investigation visit to the facility to deliver the finding. LPA met with Director, Jeannette Davis. Census was taken. There was a total of 17 preschool children with 2 staff in different classrooms.
A review of facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The complainant alleged the facility is allowing children to attend school that have been exposed to a contagious disease. According to the complainant, each day, more children are sent home due to the hand foot mouth disease. Per complainant, "I have concerns regarding them to allow children to come to school ill. For instance, this week a child was sent home for hands foot and mouth disease. They put a letter out on Monday letting parents know. Tuesday, they send students home from the disease spreading. Comes Wednesday more students get sent home because they continue to allow them to come to school”. LPA reached out to complainant via email for additional information, however no additional information was provided.

On 9/19/22, LPA interviewed 5 staff including the Director. According to the director, there were 3 cases of hand foot mouth disease confirmed by the parents which began on September 12, 2022. All 3 children were in the same classroom in the 2-3-year-old classroom. The director stated a few of the children were absent from school during the 2-week period. However, parents did not notify Director if their child had hand foot mouth disease or if they were taking precautionary steps. The children with the hand foot mouth disease were required to provide doctor’s note when they return to school. The director stated that she informed the licensee who contracted a company to disinfect the school. The 4 staff also confirmed there were cases of hand foot mouth disease. However, the staff were usure of the total number of hand foot mouth disease. 1 staff stated a few children were absent due to precautionary steps taken by the parents. All staff stated they sanitize and disinfect the whole classroom. LPA reviewed the 3 children’s file and the doctor’s note was in the file for the 2 of the 3 children. Director was unable to locate the doctor’s note for the other child and requested parent to resubmit.

On 10/18/22, LPA called 12 parents and interviewed 5 of the 12 parents. There were no disclosures made from the 5 parents interviewed. LPA did not receive a return call from the other 7 parents.

Based on interviews conducted, the complaint alleging facility is allowing children to attend that have been exposed to a contagious disease is found to be unsubstantiated. Although the allegation may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

Exit interview was conducted. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights.
