25 | Licensing Program Analyst (LPA) Tran and LPA Jung conducted a case management inspection to follow up on two self-report unusual incidents that were submitted to Orange County Regional Office on 07/26/2024 by facility staff and 07/30/2024 by Director Evelyn Rodriguez. According to the Director, on 07/26/2024, Parent #1 (P1) submitted a written letter to Director regarding a concern with allegation about Staff #1 (S1) was handling their child in a rough manner. Director met with S1 and addressed concern with S1 with a written warning. On 07/29/2024, Director reported that Parent #2 (P2) met with Director and disclosed a concern in which P2 witnessed S1 was being rough with a child in care on 07/29/2024 at approximately 10:15am when P2 dropped off P2's child at the classroom. Director once again met with S1 to address concern about the alleged incident. According to the Director, S1 resigned from position at the facility.
During today's inspection a tour of the facility was conducted. Observed at the time of inspection was 6 toddlers supervised by 1 staff in the Toddler's room, 20 preschool children supervised by 2 staff members in the preschool classroom and 5 school age children supervised by 1 staff on the playground.
A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances.
During today's inspection Director was interviewed and Director provided document regarding parent's concerns and actions taken by the facility to address the concerns.
Based on LPA’s interview and records review, no violation of Title 22 regulations being observed. The facility has taken appropriate actions to address parents' concerns/allegations.
Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Evelyn Rodriguez. |
