Licensing Program Analysts (LPAs), Mila Quinto and Nick Tran conducted an investigation visit to the facility to deliver the findings. LPAs met with Director, Evelyn Rodriguez. Census was taken. There was a total of 11 preschool children with 3 staff members. At 9am, director arrived at the facility.
A review of facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The complainant alleged the facility has mice/rodents/rats.

On 9/23/22, LPAs interviewed 4 staff members including the director. The director admitted the facility has rodent problem which started June/July 2022, however does not recall the exact date. An exterminator has been called for service on July 20, 2022 and September 13, 2022, and director states that she has seen some improvement. However, there is still an ongoing rodent problem. 1 staff stated seen 1 to 3 rodents in the kitchen a few weeks ago. 2 staff members also confirmed they have seen droppings in the kitchen and classrooms.

LPAs reviewed receipt order from Terminix dated 7/20/22 and 9/13/22 stating inspected and found rodent droppings.

On 09/23/22 while LPAs toured the toddler classroom, LPAs observed a dropping on the carpet in the middle of the classroom. LPAs also observed droppings in room 1 located in the middle classroom immediate left from the entrance.

This agency has investigated the complaint alleging facility has rodents. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.

The complainant alleged the facility is unsanitary. The complainant stated the food and floors are contaminated with droppings and urine from rats.

On 9/23/22, LPAs interviewed 4 staff members including the director. According to the director, the staff are thoroughly cleaning and sanitizing the whole facility and covering all food in a plastic bin to prevent contamination. 2 staff stated they have seen droppings in the kitchen and classroom and would sweep daily to discard the droppings. 1 staff stated have not seen rodents nor droppings.

During the tour of the facility on 09/23/22, LPAs observed rat/mouse traps in the kitchen with droppings. After nap time, LPAs toured the toddler’s classroom and observed a dropping on the carpet. LPAs also observed droppings in room 1 located in the middle classroom immediate left from the entrance.


This agency has investigated the complaint alleging the facility is unsanitary. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.

Based on LPAs observations, record reviews and interviews the following violations were observed and being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 3, Section 101238(a)(1) Buildings and Grounds. See LIC 809D.

If the facility receives a Type A violation, the licensee shall post and provide copies of the report to parents/guardians of the children in care at the facility by the next business day, and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days and provide a copy to current and enrolling parents. The licensee is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child’s file. Failure to post Type A reports for 30 days will result in a Civil Penalty of $100.00

Exit interview was conducted. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
