Licensing Program Analyst (LPA), Alanna Gontarek conducted an onsite inspection for the purpose of an Annual Random. LPA, Owner, Deborah Cosby and director, Taryn Stephens toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. Census was taken in individual classrooms. The overall census observed was 2 school age staff and 1 preschool children. Per Director, school age children are currently only coming after school from 11:30 a.m. to 6:30 p.m. Due to COVID 19 guidelines, LPA observed staff and children following CDC and Dept of Public Health Guidelines. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:00 a.m.- 6:30 p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. LPA observed medications to be stored in the office in a locked box. Cleaning compounds are stored in the school age room on an upper cabinet, and the laundry room on an upper cabinet. At 9:32 a.m., LPA observed 1 box of Raid Foggers and 1 can of Raid Ant and Roach killer on an upper shelf in the laundry room in a clear container, with no lid. Pictures were taken. Although the Raid products were inaccessible on a high shelf, facility did not lock the laundry room door with either a key or combination lock. Owner immediately discarded poisons. LPA inspected the food preparation area. Food is prepared on site; breakfast, lunch, and snacks are provided. Food prep areas were clean and sanitary. Food is properly stored. Menus were posted where they could be reviewed by parents. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. LPA observed drinking water available to children indoors by water jugs. Per Director, children bring sports bottles, from home, and are labeled with the child’s name on it. The children's bathrooms are clean and sanitary. The facility has a working carbon monoxide detector and fire extinguishers last serviced 8/20/21. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility as well. Per Director, isolation area for ill children is located in the office.

Staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization record, as required, were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a childcare center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for director and staff were reviewed and within compliance.
Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. All staff completed renewal of the Mandated Reporter Training, and is on file. Based on record review, at least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires on 9/5/2023. The facility has conducted a fire drill within the past six months, on 10/18/2021. At 9:40 a.m., LPA observed there was no Earthquake Drill documented. Per Director, facility does not have an Earthquake Drill log on record.

Children's records were reviewed, and there was a separate, complete and current record for each child in the areas reviewed the children’s files were found to be in full compliance. Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day.

Medication:

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material made of wood chips and grass around the climbing equipment, slides and other similar equipment appeared to be enough to absorbs falls. Per Director, drinking water in the outdoor activity space is provided by water jugs and disposable cups and children's labeled sports bottles (brought from home). The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.


The director was informed that Licensing Quarterly Updates are available at www.ccld.ca.gov Director receives emails for Quarterly Updates and PINs from www.ccld.ca.gov and select.

Information on the additional nutrition training, immunization requirements for children, and Health Schools Act (http://www.cdpr.ca.gov/docs/pestmgt/schoolipm.htm) were provided. The director was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative.

The following deficiencies listed on the attached deficiencies 809D page are being cited in accordance with California Code of Regulations Title 22.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Director, Taryn Stephens and Owner Deborah Cosby. The report was reviewed and discussed. Appeal Rights and violations were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The licensee did not comply with the section cited above in that Earthquake drills are not completed or documented, which poses a potential health, safety or personal rights risk to children in care.

Per Director, conducted an Earthquake Drill on 10/21/2021. The Director states she will document the drill on an Earthquake Drill form and submit the completed form via email to the Department. Per Director, will make Emergency bags, with emergency supplies, in each bag, in each room, including each child's current emergency information. Per Director, will submit pictures of an emergency bag via email to the department by the due date of 11/1/2021.
