An unannounced Annual/ Random Inspection was conducted on this day by Licensing Program Analyst (LPA) Ketki Desai in accordance with Community Care Licensing (CCL) Title 22 Division 12 Chapter 1, governing Infant Child Care Centers. Facility is currently licensed for a capacity of 20. Licensing staff met with Administrator Bernhart Kristi and Samantha Madrigal. The program currently operates Monday thru Friday from 6.30 am to 6.00PM. Licensing staff was taken on a guided tour of the facility of both indoors and outdoors. A review of staff records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance.

During the site inspection, there is a total census of 14 infants present ( Stars: 5 infants and 2 teachers / Sparklers : 9 infants with 3 teachers). During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios.
Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for infants’ belongings and napping equipment/cribs was inspected and meet all regulatory requirements. Availability of drinking water was reviewed. The children have their own sippy-cups with their names written on them. Changing table has raised sides at least three inches high and is within an arm’s reach of a hand washing sink. Toys observed to be clean.

A first aid kit is kept in each classroom. Carbon monoxide detectors and smoke detectors are present in the facility. Storage cabinet in classrooms was locked and hazardous items including poisonous cleaning compounds were stored inaccessible to infants.
Infants bring their own bedding materials but are washed daily on the premises. Storage for infant's belongings was reviewed. Food preparation areas were toured for safety, cleanliness and proper equipment. Meals are provided by parents. Bottles, dishes, and containers were labeled with infants’ names; dates were visible on all infant's bottles and other refrigerated items.

Infants bring in their own diapers, only cleaning wipes are provided by the facility. Facility communicates electronically upon the need for diapers with the parents.

Napping area was observed to have cribs with infants name, and meets the regulatory requirements. Older infants have a scheduled napping time.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. LPA Desai advised Administrator that the Infants need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Infants use their own sippy-cups with their name written on the cups when outdoors. Play area was inspected for hazards and inaccessibility to bodies of water. No hazards or bodies of water observed.

Teacher-infant ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of infants are met and appropriate. Infant Needs and Assessments are done quarterly. Sign-in and out sheets and procedures were reviewed. Personal Rights of infants were observed by LPA. Staff and Infant Records were reviewed for completeness. Inspection of required forms made.
Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. CPR card expires in 3/30/2020 for Administrator present on site. Including Immunization's and Mandated reporter training.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following was discussed with the Director: Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited. No infant walkers, no Johnny Jumpers, no excersaucers or any other item that falls into that category are allowed in facility.

The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

LPA informed Director to log onto web site www.ccld.ca.gov to obtain forms and LIVE SCAN application. Records for all children and staff must be maintained for three (3) years after separation from the facility.

A copy of the 2016 “A Child Care Providers Guild to Safe Sleep” was provided to the facility representative.

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold.

No deficiencies cited on today's inspection.

Exit interview was conducted. Report reviewed and discussed. Notice of Site Visit was posted during the visit. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door. Appeal rights provided and explained. The director was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.
