During the course of the investigation, LPA observed classroom activities, interviewed six staff members, reviewed the Two's class daily schedule, reviewed employee sign in/out sheets, reviewed personnel files, reviewed electronic system (tadpoles) and obtained a copy of the children's roster.
The complainant stated a staff member in the 2's classroom (Room 5) was observed changing a child's pull up diaper without gloves and proceeded to change the diaper of another child without washing their hands in between children. The complainant stated the staff member put on one glove during the second child's diaper change and did not see the staff member wash their hands.
The director stated staff are instructed to first wash their hands, put gloves on, get the child's diapering supplies and change child, put a new set of gloves and change their diaper, throw the diaper away, sanitize the diaper changing table, and wash hands before handling next child. A copy of the diaper changing procedures for the facility was obtained.
Staff members interviewed stated they wash their hands between each diaper change and denied the allegation. One staff member who worked in Room 5 is no longer employed by the facility and was not available for interview. The surveillance footage of the facility does not show coverage of the diaper changing area. This facility utilizes an electronic reporting system (Tadpoles) where meals, diaper changes/toileting, and naps are reported to parents. On the date of the alleged incident, there is no activity logged after 11:46 AM. The observation by the complainant was made in the afternoon. During classrooms observations, LPA observe 2 staff members washing their hands at the appropriate times.
Based on the unavailability of records (tadpoles system) to identify the staff member, one staff member no longer being employed at the facility, the lack of surveillance coverage of the diaper changing area there is no evidence to corroborate the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation: Staff failed to wash hands in between diaper changes is unsubstantiated.
Exit interview was conducted. Notice of Site Visit was posted during the visit. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. |
