32 | During the course of the investigation, LPA Torrence conducted in-person interviews with 11 staff members. Staff 1(S1) and Staff 2(S2) stated their COVID-19 protocol is that they contact the OC Public Health Department and follow whatever instructions that is given. S1 reported if a staff or child test positive for COVID, they are required to stay home until the required quarantine date, and upon returning they are required to submit a negative test. S1 stated they notify the parents and staff of any positive cases via a phone call and a letter. S2 stated if it’s a staff member which tested positive and they are at work, staff is immediately sent home, parents are sent a close contact letter and a text is also sent out, classroom is shutdown according to Public Health requirement. S2 stated positive staff members are not allowed to stay at the facility and they have never had a case where a staff is already at the facility and got sick, staff have always called in sick and tested positive while at home. S2 stated all staff members are notified of any positive cases whether it’s a staff or child. Nine interviewed staff reported they are notified of positive cases. Staff 3, 6, 7, 8, 9, and 11 stated classrooms are shutdown/closed when there are positive COVID cases.
During the course of the investigation, LPA Torrence interviewed five parents. Interviewed parents had no issues or concerns nor complaints about the facility.
Based on the information gathered from LPAs' interviews, reviewing records, there was insufficient evidence to corroborate the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is unsubstantiated.
Exit interview was conducted. The Notice of Site Visit was posted. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) will be provided through email and their signatures on this form acknowledges receipt of these rights.
