Licensing Program Analyst (LPA) Tran conducted a case management inspection to follow up on a self-report unusual incident that was submitted to CCL Regional Licensing Office on 07/07/2023. It was reported by Tracy Nguyen, Director.

According to the Director, the incident occurred on 07/06/2023 at approximately 6pm. Director stated that Staff # 1 (S1) signed out child #1 (C1) at approximately 5:55pm, when C1 was witnessed sitting at the bench in front of Director's office. At 6pm, S1 was getting ready to leave the facility with C1. Director walked out of the office to ensure front gate of the facility was locked. Within a couple of minutes, S1 realized that C1 was no longer on the bench in front of the Director office. Director and S1 went to look for C1 in all of the classrooms and facility grounds. C1 was no where to be found. Director called Garden Grove Police Department (GGPD) and GGPD arrived approximately at 6:30pm, and found C1 in Hope classroom, which adjacent to the Director's office. No Police Report was generated.

During today's inspection, LPA Tran informed Director of the purpose of the visit. A tour of the facility was conducted, observed at the time of the visit was 51 preschool children with 6 staff members. A review of staff records on this date 07/11/2023, indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances. During today's inspection, LPA obtained attendance chart (sign/out sheet of the date of the incident), LPA interviewed Director and reviewed camera footage of the incident. S1 and C1 were absence during today's visit.

Due to insufficient information available at this time and further interviews needed, the above allegations need further investigation.

Exit interview was conducted. The Notice of Site Visit was posted. Director was advised the Notice of Site Visit must be posted for 30 days or 100$ Civil Penalty will be assessed. (End of Report)
