An Informal Meeting was conducted on this day in the Orange Regional Office. Present during the meeting were Licensing Program Manager (LPM) Rina Lopez, Licensing Program Analyst (LPA) S. Hawkins, Licensing Program Analyst (LPA) Eileen Corral, and licensee Joy Thomas.

The purpose of this informal meeting is to discuss an incident which occurred in 2017 regarding the welfare of a child.

7/27/17 Personnel Requirements - 101216(a) Child care center personnel shall be competent to provide the services necessary to meet the individual needs of children in care and shall at all times be employed in numbers of sufficient staff to meet those needs.
-Staff failed to meet a student's needs when they had knowledge of the child's behavior. Staff failed to follow their own protocol, they left one staff alone with a student which allowed a situation to occur and resulted in an injury to the student. This compromised the welfare of the student.

The facility representatives states that all staff have received crisis intervention training and they will ensure compliance with Title 22 Personnel Requirements 101216(a) . The licensee stated they continue to conduct monthly safety training, increased supervision and maintain ratios.
Licensee expressed the need to diversify her program to possibly provide services to younger children to meet the needs of the community and family served.

· Always place infants on their backs for sleeping


· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold

The following was also discussed with the Licensee:


I. The facility will be placed on more frequent visits for the next year.
II. The licensee was advised that it is her responsibility to know & understand Title 22 Regulations.
III. The licensee's facility must be in compliance at all times.
IV. The licensee was advised to check the Child Care Licensing web site at www.ccld.ca.gov for quarterly updates, training, forms and regulations.
