Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of decreasing capacity from 8 infants and toddlers to 12 infants and toddler by expanding the existing room. LPA met with director, Avigail Mizrahi (Lynne Pinto) who toured the facility with LPA. Census was taken. Currently the facility is licensed for 8 infants and toddlers in room # 3. The licensee has requested to provide care and supervision for 12 infants and toddlers in room # 3 for ages 6 weeks old to 24 months old, Monday through Friday, 8:00 a.m. to 6:00 p.m. Census was taken. There were 4 staff with 6 infants and toddlers. Napping area was inspected with 8 cribs in it .

A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The parents provides milk, food, diapers, beddings for this program. The children are dropped off and are singed in and out at the entrance of the classroom. The facility has met all the posting requirements. LPA checked the log for every 15 minutes check on infants and toddlers. Infants needs and service plan were reviewed. LIC 9227 for sleeping plan was discussed.

The indoor activity space measurement for infant/toddler room # 3 is as follows:
Room # 3 : 833 square feet.
Total indoor space 745.5 square feet divided by 35 = 21.3 (21 children)
Toilets: 1 X 15 = 15 children
Sinks: 2 X 15 = 30 children
Sleeping area which was is fenced, deducted from the activity measurement: 247 square feet
Outdoor space on the preschool playground is fenced but it will be expanded. The director stated they are planning to expand the fence to the end of the grass area. LPA measured the extended grass area which was not fenced yet and it was enough for additional 5 children.
Total outdoor activity space measurement is as follows:
Outdoor Infant/toddler space located on the preschool playground: 1012.5 square feet divided by 75 = 13.5 (13 children).
According to the measurement of the preschool playground dated 7/20/2017, The outside activity area was measured 7106.6 square feet divided by 75 = 94 children (The preschool of this combination program is licensed for 45 children).
The facility is equipped with carbon monoxide detector.
LPA discussed the safe sleep regulations with director, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with director the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Orange county Fire authority has approved the requested capacity of 12 infants and is on file.



Based on today’s indoor and outdoor measurements, sinks and toilets availability, the request of the facility for increasing the capacity to 12 infants and toddlers will be final reviewed and discussed with management in the Regional Office. The facility will be notified if any additional document is required.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Exit interview conducted and report was reviewed with the director, Lynne Pinto.

The following are needed before an updated license is issued: Please send them no later than 30 days of today's date.

1- Expansion of the playground for infants and toddler to the end of the grass area by extended fencing.

2- Rubber mats around the falling zone of the low high slide which is located on the grass.
