Licensing Program Analyst (LPA), Yesenia Villa conducted an onsite inspection for the purpose of an Annual Random. LPA and Anuradha Tasildar, Director toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. Census was taken, there were twenty-six preschool children present upon arrival with four staff. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The facility was observed to be operating within its licensed capacity and within compliance of staffing ratios. The facility operates from 7:30am to 06:00 pm, Monday thru Friday. The children are from ages 2-6 years old. The facility has three classrooms labeled as the land room, the water room and the air room. As stated by the Director the children are designated to each classroom depending on the child’s age. The water room is specifically designed for the younger children.

A children’s roster was not available during this inspection. The center had a list of the children’s name and a separate list. LPA Villa explained to Ms. Anu that a children’s roster must be kept on site aside from the LIC700. LPA Villa explained that a compiled list with the children’s contact information, doctor, parent/emergency and enrollment date and end date is required. LPA Villa explained that the center can use the LIC9040 or compile it on their own list if the information is on the list. The director understood this requirement. During the inspection of the indoor activity space, items which could pose a danger to children detergents, cleaning compounds, and medications were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Lunch is brought from home as stated by Director. Menus were posted where they could be reviewed by parents. The center offers, AM and PM snacks and an inspection of the supplies was conducted. Director indicates parents donate snacks to the school and the remaining of the snacks are purchased by the Director. The floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges.

There is drinking water available to children indoors via drinking fountains and children bring their own water. The children's bathrooms were observed to be clean and sanitary. The facility has conducted an emergency drill within the past six months. The facility has a working carbon monoxide detector, smoke alarm and fire extinguisher. The smoke alarm and carbon monoxide alarms were tested during today’s inspection. Facility met all the posting requirement.

Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for all staff were reviewed and were within compliance.

Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years, this requirement was observed not within compliance for all seven personnel staff. It was observed during file review that two of the personnel staff had the School Personnel Mandated Reporter training completed. LPA Villa informed Director that all staff must complete the correct Mandated Reporter training in compliance with (AB1207) and provided the web site for completion.

At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 11/01/2021. Director was informed that the opener and the closer must also posses an unexpired, EMSA approved Pediatric CPR/First Aid certification to ensure the safety of the children in care. Children's records were reviewed, and there was a separate, complete and current record for each child. Children’s records were reviewed for LIC700, LIC627, and LIC701. In the areas reviewed the children’s files were found to be in full compliance. Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day.

This facility does not provide Incidental Medical Services - (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at
least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. There was cushioning material to observe a fall. Drinking water in the outdoor activity space is provided via water fountain and the children’s own cups. The outdoor equipment and toys were in good repair and free of sharp edges.

Inspection and report reviewed and exit interview was conducted in with Director, Anuradha Tasildar.

Information on the additional nutrition training, immunization requirements for children, and Health Schools Act (http://www.cdpr.ca.gov/docs/pestmgt/schoolipm.htm) were provided. The Director was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative. Based on LPAs observations and record reviews the following citations will be cited on the LIC809D page attached, in accordance with Title 22, Division 6, Health and Safety code 1596.841, Current roster of children provided care in facility required. Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician.

An exit interview was completed with Director Anuradha Tasildar. The report was reviewed and discussed. Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
