25 | Licensing Program Analyst (LPA), Connolly, conducted an unannounced annual inspection of the facility on today's date. Upon arrival the LPA met with director Kristaloi Bartolome who accompanied the LPA on a tour of the facility. Census was taken. There were 45 children present with nine attending staff. A review of criminal clearance records on this date indicates adults who require caregiver background checks have received criminal record and child abuse index clearances.
Operating hours are 7am-6 pm, Monday through Friday. Rooms 2, 3 and 4 are designated for use by preschool children. Room #1 is designated for use by children enrolled in the toddler option program.
Preschool ages are 2 to 6 years and toddler option is 18 months to 36 months.
LPA inspected the following areas: availability of drinking water, age appropriate sinks and toilets, water temperature, toilet paper, paper towels, medication policy and furniture & equipment, food preparation areas, cleaning and food supply storage areas, outdoor equipment (safety, cushioning material, good repair, and age appropriateness, required shade, drinking water and fencing), teacher child ratios (staff names recorded), care and supervision, sign in and out sheets, verification of current CPR/First Aid.
LPA was informed by director that this facility provides Incidental Medical Services (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
The director provided updated personnel report, updated disaster plan and record of practised disaster drills. The LPA provided information regarding lead poisoning.
After a tour of the center and review of children's and staff records, no deficiency was observed.
Exit interview was conducted. The Notice of Site Visit was posted. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. Appeal Rights were provided. |
