On 09/16/2020, our office received a complaint and complainant reported that Facility is operating over ratio and infant sleeping in the crib is left unsupervised.
LPA Nguyen conducted interviews with 2 staff members. No disclosures were made by staff. All interviewed staff members denied of facility is operating over ratio or children are not adequately supervised during nap time. No staff member disclosed witnessing any staff not adequately supervising the infants during nap time or being left alone with more than 4 infant or 6 toddler option children. Interviewed Staff indicated that staff members always stand at the position where that they can supervise all infant during naptime. Staff members also denied witnessing any staff members leaving the children napping alone in the napping area. A staff member stated if an infant fell asleep, the staff member would call the director to come supervising the napping infant so the staff member would supervise the other infants. Interviewed staff members also stated there are only 4 infants enrolled in the infant program so there was no way the facility is operating over ratio.
On 3/8/21 LPA Nguyen conducted an inspection at the facility and observed one staff member supervised 3 infants.
No children interview was conducted due to children being non-verbal.
LPA Nguyen attempted to interview five parents and was able to interview four parents. Interviewed parents stated they like the facility and all staff members are very nice; they never witness the facility operating over ratio or observing staff members left the child unsupervised during naptime. Per interviewed parents, they did not have any concern or issue with the facility.
Based on the information gathered from LPAs' interviews and observation, there is insufficient evidence to corroborate the allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged facility is operating over ratio and children are not adequately supervised during nap time did or did not occur, therefore the allegations are UNSUBSTANTIATED.
Exit interview was conducted. The Notice of Site Visit was not posted due to tele-investigation Covide-19 State of Emergency. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) will be provided through email and their signatures on this form acknowledges receipt of these rights. First level appeal is to Regional Manager, address is above on the report.
