A pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Ho to inspect and measure facility for capacity determination. Meet with CEO Katelyn Le. The proposed program will operate from: Monday to Friday 7am to-6pm. Applicant is seeking to provide care for 90 preschool children. LPA toured the entire facility indoors & outdoors and the following measurements were taken:
Indoor:
Room 11: 23' x 21'6" = 494'/35' = 14 children.
Room 12: 22'6 x 20'6" = 461'/35' = 13 children.
Room 13: 20' x 21' = 420'/35' = 12 children.
Room 14: 22'6 x 21' = 472'/35' = 13 children.
Room 16: 31' x 25'9 = 798'/35' = 23 children.
Room 17: 20'3 x 20'6 = 415'/35' = 12 children.
Room 19: 16'6 x 29'6 = 486.75'/35' = 14 children.

Total indoor activity space: 101 children

Total children toilets and sinks:
8 toilets x 15 = 120 children
10 sinks x 15 = 150 children

Outdoor:
45'3 x 53'6 = 2420'
37'6 x 78'9 = 2958.75'
Total outdoor activity space: 5378'/75' = 71 children.
Waiver to use staggered schedules for outdoor play has been submitted.

Applicant was notified about disaster drills, posting requirements, children records, mandated child abuse and injury/ death reporting, SIDS and Never Shake a Baby. Applicant was also advised, once licensed, the Notice of Site Visit must be posted for 30 days and if serious violations are cited then the Licensing Report (LIC809 or 9099) must be posted by the Notice of Site Visit for a period of 30 days or $100 civil penalties will be assessed. Applicant has been informed that all employees must be associated to the facility. If the licensee fails to have proof of a fingerprint clearance or fails to associate a previously cleared individual to the facility, a civil penalty of $100.00 per day per person will be assessed. The first violation is subject to the penalty for up to five days. If there is a subsequent violation within a 12 month period, the fine will continue for up to 30 days.

Facility meets all licensing requirements and file will be submitted for approval as of today.

Exit interview was conducted and a copy of this report was provided to the director on this date.

The facility representative was informed that the CRIMINAL RECORD STATEMENT (LIC 508) has been updated, and the facility must now use the new form with revised date 7/15. Mrs. Le was also informed that the LIC 508 must be submitted with all Criminal Background Clearance Transfer Request (LIC9182).

The applicant was also informed to visit the www.ccld.ca.gov website for Quarterly Updates.

Information on the additional nutrition training, immunization requirements for children, and Health Schools Act (http://www.cdpr.ca.gov/docs/pestmgt/schoolipm.htm) were provided. The facility representative was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org and a copy of the 2016 “A Child Care Providers Guild to Safe Sleep” was provided.

Fire clearance from Garden Grove Fire Department has been received and approved for the requested capacity.

Base on today’s measurement and the sink & toilet availability, the capacity is limited to 101 children. Center has sufficient activity space to support the requested capacity of 90 children.

The following was observed:
· Classrooms are adequately equipped with age and size appropriate furniture and equipment.
· Water jugs will be available for inside & outside drink.
· Playground is enclosed by appropriate fence.
· Outdoor activity area is supplied with age and size appropriate equipment.
· Adequate shade is provided by awning.
· Food preparation area is equipped with refrigerator; sink with hot and cold running water;
storage area; utensils; and adequate amount of food supplies
· Teacher lounge room by the office area will temporarily serve as the isolation area for ill children until
parents arrive.
· Staff bathroom will also be used as the isolation bathroom.
· Medication will be stored in the classroom's cabinet and is inaccessible to children.
· Medication administration forms were reviewed.
· First Aid kit is complete.
· Sign in/Sign out record was reviewed and meets regulation requirements
· There is sufficient napping equipment, and bedding will be laundered on a daily basis/sent home weekly to
wash.
. Facility has working smoke detectors, carbon monoxide detectors, and fire extinguishers.

LPA discussed Incidental Medial Services (IMS) with director. This facility will provide Incidental Medial Services (IMS). The updated Plan of Operation will be submitted to LPA within 30 days.
