A pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Ho to inspect and measure the Early Childhood Educator Setting for capacity determination. Meet with Director Berverlee Mathenia. The proposed program will operate from: Monday - Friday 6am -6pm. Applicant is seeking to provide care for 12 infants. LPA toured the entire Early Childhood Educator Setting indoors & outdoors and the following measurements were taken:
Indoor:
Classroom 2: 34'9" x 15'9" = 547'
9' x 16'3 = 146'
Total: 547' + 146' = 693'/35' = 19 children.

Total children toilets and sinks:
3 potty chairs x 15 = 45 children
2 sinks x 15 = 60 children

Outdoor:
Front yard: 160' x 127' = 20,320'
57' x 94' = 5358'
Total: 20,320' + 5358' = 25,678' divided by 75' = 342 children.

Waiver for infant to share the front yard with the preschool using staggered schedules has been submitted.

Base on today’s measurement and the sink & toilet availability, the capacity is limited to 12 children. Center has sufficient activity space to support the requested capacity of 12 children.
Fire clearance from Orange County Fire Authority has been received and approved for the requested capacity.

of Site Visit must be posted for 30 days and if serious violations are cited then the Licensing Report (LIC809 or 9099) must be posted by the Notice of Site Visit for a period of 30 days or $100 civil penalties will be assessed. Applicant has been informed that all employees must be associated to the facility. If the licensee fails to have proof of a fingerprint clearance or fails to associate a previously cleared individual to the facility, a civil penalty of $100.00 per day per person will be assessed. The first violation is subject to the penalty for up to five days. If there is a subsequent violation within a 12 month period, the fine will continue for up to 30 days.

Facility meets all licensing requirements and file will be submitted for approval as of today.

Exit interview was conducted and a copy of this report was provided to the director on this date.

Information on the additional nutrition training, immunization requirements for children, and Health Schools Act (http://www.cdpr.ca.gov/docs/pestmgt/schoolipm.htm) were provided. The facility representative was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org and a copy of the 2016 “A Child Care Providers Guild to Safe Sleep” was provided.

LPA reviewed with licensee the following safe sleep best practices:

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot
or too cold.

The following was observed:
. Nine cribs located in a napping area.
. Changing table is within arm's reach of a sink and will be sanitized after each use.
. Three potty chairs and 2 high chairs.
· Classrooms are adequately equipped with age and size appropriate furniture and equipment
· Water fountain will be available for inside & outside drink.
· Playground is enclosed by appropriate fence.
· Outdoor activity area is supplied with age and size appropriate equipment
· Adequate shade is provided by 2 Pop-up Canopies.
· The Nurse Office area is located by the main entrance and will temporarily serve as the isolation area for
ill children until parents arrive. There is a bathroom in the Nurse Office and it will be used as the isolation
bathroom for ill children.
· Medication will be stored in the classroom cabinet and is inaccessible to children.
· Medication administration forms were reviewed.
· First Aid kit is complete.
· Sign in/Sign out record was reviewed and meets regulation requirements
· Bedding will be laundered on a daily basis
. Facility has working smoke detectors, carbon monoxide detectors, and fire extinguishers.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
