LPA Blesi met with Director Denise Schleth for a case management visit on this date to discuss the unusual incident report (UIR) that she submitted on 5/28/2019. There were 24 preschool children present and 4 toddlers participating in the toddler option.

LPA interviewed staff and director regarding the incident.


No deficiencies were cited during today's inspection.
