Licensing Program Analyst (LPA), Amanda Blesi, met with Director, Denise Schleth, via Google Duo due to the COVID-19 pandemic, for a case management inspection to discuss the unusual incident report (UIR) that the director submitted on 4/26/2021. Two staff, the director and two children were interviewed regarding the UIR.

An exit interview was conducted, and Notice of Site Visit was provided.

Facility evaluation report was emailed to director and an email verification of receipt of report will be used in lieu of a signature on this report.
