On August 17, 2023, Licensing Program Analyst (LPA) Lea Habtom met with Site Supervisor, Sofia Ramirez, for the purpose of a required annual inspection. This facility is located on Lincoln Crossing Elementary School Campus. The facility utilizes the multipurpose room and the gym. There was a census of 6 transitional kindergarten children present supervised by one staff and the site supervisor. All criminal record clearances are associated to OAKHILLS STAR.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA toured the classroom, outdoor play area and restrooms. The facility uses the school playground and restrooms for the children. Staff stated there are no children who require medications. Toxic and hazardous items are inaccessible to children. Furniture and equipment are in good condition. Playground equipment and surfaces are free of loose or sharp parts. Facility shares the school restroom. The floors appeared clean throughout the facility. All food was protected against contamination. Children bring their own snacks and staff have extra snacks available. Drinking water was readily available to children both indoors and outdoors. Children bring their own labeled water bottles and the facility has a water pitcher. There are no firearms or bodies of water on the premises. LPA observed a functional carbon monoxide detector. Staff and children files were reviewed. At least one staff member present today has current Pediatric CPR and First Aid certification. LPA reviewed the Department's inspection authority and discussed any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Report continued on 809-C.
