During the investigation, LPA Habtom conducted interviews and observation regarding the allegation that staff did not keep information regarding a child confidential when reporting incidents. The reporting party showed concern that providing other children names to other parent’s boo boo reports can cause retaliation or an uncomfortable environment. LPA Habtom reviewed a sample of boo boo reports provided by the director and found 1 with another child’s name. LPA L. Habtom was unable to determine that providing other children names is a common practice. Interviews with staff and parents revealed that no other children names are stated on the boo boo reports. Based on the information gathered, LPA Habtom concluded that the allegation staff did not keep information regarding children confidential to be UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the allegations did or did not occur.
Staff did not meet child's toileting needs
During the investigation, LPA Habtom conducted interviews and observation regarding the allegation that staff did not meet child’s toileting needs. The reporting party expressed concern of a child coming home in clothes that was urinated in. Staff interviews indicated that children are changed when noticed and that the intentions are to not have the child sit in urinated clothes. LPA Habtom observed that the children used the restroom before outside play and circle time. A child also had an accident who was changed by staff immediately. Based on information gathered, LPA Habtom concluded that the allegation staff did not meet child’s toileting needs to be UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the allegations did or did not occur.
No Title 22 regulations were cited during today's inspection. This report was reviewed with director, Dorothy Oerly. A notice of site visit was provided to be posted for 30 days.
