On March 5th, 2020 at 11:30 am, Licensing Program Analyst (LPA) Jeremey McClain met with Director Malkie Korik for an unannounced annual random inspection.

Around 11:40 am, LPA observed a census on the outside playground of 12 children, which included six toddlers and six preschool children. The children were supervised by three teachers at the time. LPA observed a census of seven preschool children supervised by one teacher in another room around 11:50 am.

Facility hours of operation are Monday through Friday from 9:30 AM to 4:00 PM.

LPA toured all activity and classroom spaces, restrooms, food service, and outdoor play areas. Medications are appropriately stored and inaccessible to children. Toxic and hazardous items are inaccessible to children. Furniture and equipment are in good condition. Playground equipment and surfaces are free of loose or sharp parts. The areas around or under climbing equipment are cushioned with bark to absorb the fall. Toileting facilities are in safe, sanitary and operating condition. The floors appeared clean throughout the facility. The food preparation space is free of litter and all food was protected against contamination. Storage containers with solid waste have tight-fitting covers. Program provides a snack and lunch. Menus were posted and drinking water was readily available to children both indoors and outdoors. LPA observed full legal signatures while reviewing the sign in and sign out sheet for toddlers but did not observe them for preschoolers.

Children's records were reviewed electronically. Each child's file contained an emergency card and a medical assessment. LPA attempted to review personnel records at the facility today, but only two personnel records were produced during today’s inspection.

A functional carbon monoxide detector was observed.

Incidental Medical Services (IMS) policy was discussed. The facility is not currently providing IMS. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

LPA reminded the Director that the Mandated Reporter Training must be completed by all staff once every two years, and that the training can be found at: http://childcare.mandatedreporterca.com/



LPA informed the Director about Assembly Bill 2370, which will require licensed Child Care Facilities to test their water for excessive amounts of lead. Testing will be required beginning January 1, 2020.

LPA encouraged the Director was encouraged to the visit the Department's website at WWW.CDSS.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining child care centers.

LPA also encouraged the Director to sign up for the Child Care Advocates quarterly newsletter.

This facility evaluation report was reviewed and discussed with the Director. A Notice of Site Visit was provided and should remain posted for a period of 30 days for parental review.



In the areas that were evaluated, deficiencies were observed and are cited on the following page of this report. Appeal rights were provided and discussed with the Director.

This requirement was not met. During file review for staff, only two staff files were produces (S1 and S2). Director stated that they were unable to provide staff files for S3, S4, S5, S6, and S7. This is potential risk to the health and safety of children in care.

Preschool Program with Toddler Component. a) Licensees serving preschool-age children may create a special program component for children who are between 18 months and 36 months of age....(2)The toddler program shall be conducted in areas physically separate from those used by older or younger children. Space planning and usage for the toddler component shall be governed by the provisions of Section 101438.3. Plans to alternate use of outdoor play space must be approved by the Department.

This requirement was not met as evidenced by LPAs observation of toddler children and preschool age children using the same playground at the same time. LPA observed this while conducting a census at 11:40 am. At the time, six toddler age children and six preschool age children were on the playground with S2, S3, and S3. This is a potential risk to the health and safety of children in care.

This requirement was not as evidenced by LPA's observation of the bedding storage in the closet in the toddler room. LPA observed bedding stacked on top of each other and touching while in the closet. This is a potential risk to the health and safety of children in care.

S6 stated that the room did not have a sign in and sign out sheet today when asked by the director. This is a potential risk to the health and safety of children in care.
