13 | An unannounced inspection was conducted today by Licensing Program Analyst Owens and Gallo. LPA's met with Director, Michelle Wise. The purpose of the inspection is to open a complaint investigation. Present at time of inspection were 31 preschool children and 6 staff. Interviews were conducted and a tour of the classrooms and playground were conducted at time of inspection.
Based on observation and interviews the center playground does have a piece of play equipment that is is disrepair. The play equipment is a square box with a plastic doom on top of it that has a crack it in, the plastic is split separating from each other that can be a risk to a child. The play structure is technically called a Tube Junction Cube. There is also a self made wooden house / box on the play ground that has rusted nails that are coming out of the splinter wood / the roof part of the house - box.
The preponderance of evidence standard has been met during this investigation, therefore the above allegation is found to be SUBSTANTIATED. Violations of the California Code of Regulations, Title 22, Division 12 & Chapter 3 are being cited on the attached LIC 9099 D.
LPA Owens informed Director, Michelle Wise, that this report dated 10/18/2022
Type A citation which shall be posted for 30 consecutive days as there is immediate
risk to the health, safety, or personal rights of children in care. Also, LPA Owens informed the Director to provide a copy of this licensing report dated 10/18/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed
Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LIC 9224 was given to Director at time of inspection. Appeal Rights and Notice of Site visit given at time of inspection.
